PRIVACY POLICY

Soleil Health complies with the Privacy Act 1988 and state or territory legislation that governs how health service providers should handle personal information, including but not limited to medical information.

Soleil Health is committed to ensuring that any personal information we collect is obtained lawfully, transparently and with your consent, whenever it is practical for us to do so.  By providing personal information to us, you consent to us collecting, using and disclosing your personal information as described in this Policy.

We will usually collect your personal information directly from you or by email, telephone or via our online booking providers. Where it is not reasonable or practical for us to collect this information directly from you, we may need to collect this information about you from a third party. We may collect information from a third party where your health may be at risk, and we need your personal / medical information to provide you with emergency medical treatment.

The third parties from whom we may collect your personal information include:

  • Other health service professionals (hospitals, clinics, allied health, pathology and radiology practices) who may be involved in your care
  • Your nominated responsible persons (Next of kin / carer)
  • My Health Records, if you have chosen to participate

The personal information that we collect will include information that is necessary or required to provide you with medical services and / or for administrative purposes related to the medical services that we provide.

The personal information that may be collected include:

  • Name, age, gender, date of birth, contact details
  • Information that is related to your lifestyle and medical history (ex. Medications, family medical history, diagnostics tests and treatments, correspondence to and from your healthcare providers).
  • Medicare number or private health insurance details, for billing purposes.
  • Payment details, for billing purposes
  • Other information, including religion and ethnicity, that may be relevant to the medical services that we provide.

Your personal information is stored securely and is only shared with your consent or as required by law.

In addition, we use de-identified personal information to conduct activities related to quality assurance / improvement processes, accreditation, audits, risk and claims management and staff training.

Your personal information is also used to send you standard reminders with regards to your health (for example appointments for follow-up care). Reminders are sent via SMS, email, mail and phone calls to the number or address that you have provided to us.

When providing medical services to you, we may disclose your personal information to trusted third parties, including

  • Healthcare service providers involved in your care
  • Registries, Statutory bodies and other third parties, requested to do so by yourself or as required by law

My Health Records – if you choose to participate in My Health Record program operated by the Commonwealth Department of Health, we may access the personal information it contains. We may also disclose your personal information by uploading your health information electronically to the My Health Record portal. If you choose not to allow us to access your personal information stored in My Health Record, please modify access controls within the system.

We take the protection of your personal information seriously and take all reasonable steps to ensure the information we collect is accurate, secure and protected from misuse and loss and from unauthorized access.
